This procedure explains how to raise a concern or complaint about how Abigail Strachan, trading as The Endo OT, handles your personal information, and what you can expect to happen next. It reflects the requirements of the Data (Use and Access) Act 2025 for handling data protection complaints.
1. How to raise a complaint
If you have any concerns about how your personal information has been collected, used, or stored, please contact me directly in the first instance:

Please include as much detail as you can, such as what the concern relates to, when it happened, and what outcome you're hoping for. This helps me look into it properly and respond fully.
2. What happens next
Acknowledgement: I will acknowledge your complaint within 30 days of receiving it
Investigation: without undue delay, I will take appropriate steps to investigate the complaint, which may involve checking records, processes, or contacting relevant service providers
Keeping you informed: I will keep you updated on progress if the investigation takes some time, including letting you know if there is a delay and why
Outcome: I will tell you the outcome of my investigation and what action, if any, I have taken as a result
Any complaint involving personal or health information will be handled confidentially, and only shared with others where necessary to investigate or resolve it (for example, a relevant service provider if the concern involves how they've handled data on my behalf).
3. If you're not satisfied with the outcome
If you remain unhappy after receiving my response, you have the right to raise the matter with:
The Information Commissioner's Office (ICO), the UK's independent regulator for data protection, at ico.org.uk or by calling 0303 123 1113
The Health and Care Professions Council (HCPC), if your concern relates to my professional conduct as a registered Occupational Therapist, at hcpc-uk.org
You do not need to complain to me first before contacting the ICO, but I'd always welcome the chance to resolve things directly wherever possible.
4. Review
This procedure will be reviewed periodically to make sure it remains accurate and reflects how The Endo OT operates.
